You are receiving this message because Boulder County has received Moderna COVID-19 vaccines and CU Boulder has confirmed that you are eligible to receive one. Your eligibility is based on and has been confirmed by Human Resources.
Please schedule your appointment as soon as possible if you want to receive a Moderna COVID-19 vaccine at the clinic organized by Boulder County at the Longmont Farmer's Market. Vaccine appointments will be available on a first-come, first-served basis. Boulder County has received a vaccine shipment and appointments are available on Thursday, April 22, Tuesday, April 27, and Thursday, April 29. Those who are unable to make an appointment during the allotted time frame will need to wait for the next vaccine availability.
You will be automatically notified each time vaccines arrive at CU Boulder or Boulder County until you receive a vaccine or you opt out of being notified about vaccine opportunities on the .
To schedule your vaccine appointment

- Select your desired appointment time and click on ’Save and Continue’.
- On the next page, enter your personal details and click on ‘Save and Continue’.
- On the 'Health Insurance' page, you are only required to specify whether you have private health insurance, or if you receive Medicare/Medicaid assistance. Select the relevant option and click ‘Save and Continue’.
- The Health Questions section requires you to answer specific questions pertaining to your medical history. After you have answered all the questions, click on 'Save and Continue’.
- On the Consent for Services page, please select the box next to Moderna COVID-19 Vaccine. Next, sign your name in the designated box and identify your relationship to the vaccine recipient. Enter your First and Last Name, then click on ‘Save and Continue’.
- On the next page, review your submission thoroughly and then click on ‘Save and Continue’.
- Once your appointment is scheduled, you will receive a confirmation message which will tell you where to go to receive your vaccine.
What to bring to your appointment

- Bring personal identification to your scheduled appointment. You may be asked to show your employer-provided name badge/credentials (such as your Buff OneCard), or personal identification such as a driver’s license.
- If possible, please bring a paper or electronic version of this email with you to your appointment.
What to expect during your appointment
- The appointment will take approximately 30-60 minutes.
- Once you receive the vaccine, you may be asked to wait in an observation area for 15-30 minutes, depending on your medical history.
- At the appointment you will be automatically scheduled for your 2nd dose 4 weeks out, please note these three 2nd dose clinics will take place the week after finals and graduation.
Boulder County is eligible to receive the , , and vaccines at no cost to you, and no insurance information is needed.
Boulder County is unable to control which vaccine will be received at any given time. We encourage you to talk with your primary care provider if you have questions about the vaccines.
If you are able to receive a vaccine through your primary care provider or other health care facility before you are notified by CU Boulder, we encourage you to take advantage of your first opportunity.
