CU Boulder records indicate that you have not completed your COVID-19 immunization reporting requirement.听
To be eligible for the one-time $1,300 October retention payment announced by campus leadership on Oct. 3, the vaccine reporting requirement must be fulfilled by Oct. 12, 2022.
To be eligible for the base-building increases effective January 2023, the vaccine reporting requirement must be fulfilled by Dec. 31, 2022.
To comply with CU Boulder鈥檚 updated COVID-19 vaccine requirement, members of the campus community are required to have received the initial series of vaccine plus one booster, or file for an exemption.
There are two ways to complete the vaccine reporting requirement in the
1.听Submit proof of vaccination (original dose and booster)
o 聽 Input COVID-19 vaccine dose dates
o 聽 Upload COVID-19 vaccine card
2. Submit a medical or moral/ethical vaccine exemption
o 聽 An exemption can be submitted even if you had previously received a COVID-19 vaccination.听
Once you have submitted vaccine documentation or an exemption, the information is manually verified. The verification process may take 7-10 days. If you have incomplete documentation or if additional information is needed, the immunizations team will contact you directly. You will receive a secure message once the exemption has been verified.
