Forms for Scholars in J-1 Status
Requesting a Travel Signature
Designated sponsors for the Exchange Visitor Program may now digitally sign and electronically transmit the Certificate of Eligibility for Exchange Visitor Status (J-Nonimmigrant) (Form DS-2019).
To request a travel signature, complete the DS-2019 Reprint Request e-form (available in the J-1 Scholar Services section of the MyISSS portal). A new DS-2019 with a travel signature will be issued and emailed to you.
Be cautious and only submit personally identifiable information to ISSS via a method that is secure. (Note: Email is NOT secure!)I

- If you only have limited services access, contact your assigned ISSS advisor to request a link to the e-form.
Departmental Services
When required, your department administrator will initiate e-forms that appear in Departmental Services and you will recieve an email with a link to access the e-forms. You will not be able to directly access e-forms in Departmental Services.
